"In consideration of Loren Williams providing services and equipment related to guided fly-fishing, I agree to the following: I am fully aware that risks and dangers exist while using fishing equipment and participating in the act of fishing, and that my participation in the act of fishing and the use of fishing equipment may result in injury or illness including, but not limited to bodily harm, disease, or death. I understand that these dangers may be caused by the negligence of Loren Williams, the negligence of myself, the negligence of others, accidents or forces of nature and other causes. Since these risks may arise from foreseebale or unforeseeable causes, my willing participation in this activity and use of related equipment demonstrates my assumption of all risks and dangers and responsibilty from all losses or damages whether caused in whole, or in part, by the negligence of Loren Williams, or by any other person. By booking a fishing trip with Loren Williams, I hereby acknowledge that I have read and understand the above and agree to release, waive, discharge, hold harmless, defend, and indemnify Loren Williams from any and all claims, losses or actions from any damages that may arise out of my use of equipment or services provided by Loren Williams. Signature acknowledges that I have read, understand, and agree to the above." |
